If physicians have received hospital credentialing through Medical Provider Resources, they do not need to complete a membership application. They only need to email their name and their practice/group’s name, address and phone number to membership coordinator Rhonda Welch at [email protected]. Physicians can also check the MSSC membership box on the MPR credentialing application form.
If you have not been credentialed by Medical Provider Resources, please complete the membership application.
